  1. Non-Pharmacy Survey Entries
    • During the Sweepstakes Period, if you have got cash register receipts for purchases of non-pharmacy products in excess of $10, you will be offered the opportunity to take a customer satisfaction survey (the “POS Survey”)
    • Simply click the "Go to Win" button to visit the survey page.
    • Follow the onscreen instructions and complete the survey.
    • You can also call 1-800-308-7528 to complete the POS survey.
  2. Pharmacy Survey Entries
    • If you have got cash register receipts for prescription purchases from Meijer Pharmacy cash registers,  you will be offered the opportunity to take a customer satisfaction survey(the “Pharmacy Survey”). calling 1-800-308-7528
    • click the "Go to Win" button to visit the survey page.
    • Follow the instructions and complete the survey.
    • You can also call 1-800-308-7528 to complete the Pharmacy Survey.
  3. Enter by mail
