All you need to do is share your feedback in La-Z- Boy product survey and you'll be automatically entered into the prize draw. Prize winners will be notified by telephone and/or mail and/or e-mail collected at the time of entry.

Rules:

• Legal residents of the 50 United States, the District of Columbia and Canada (excluding Quebec) who are 18 years of age or older are eligible to participate.
• There will be 4 entry periods. Limit 1 entry per person per Entry Period.
• Each prize winner (U.S.) will each receive a 1099 tax form for the value of the prize.
